- Call us first with all problems unless you are experiencing a medical emergency.

- Prepare for and keep scheduled appointments or reschedule visits in advance if able.
- Bring all medications to your appointments.
- Be honest about your history, symptoms, and other important information about your health (including other doctors or clinics you may have visited).
- Take your medicine as prescribed and follow your treatment plan.
- Ask questions, share your thoughts and feelings, and be part of your care.
- End every visit with a clear understanding of our joint expectations and treatment goals.
